As soon as they are delivered to Mead Public, we offer a supply of widely used state and federal income tax forms and instructions to the public. They are available free, while supplies last, in the lobby of the library and are put out as they are received from the government agencies. (The state forms usually aren't sent to the library until some time in January.)
Only personal (not business) forms are available. Multi-part forms also are not available. Less commonly used forms are available for photocopying on the second floor of the library. All forms are available on the Internet.
Library staff members may not provide any tax advice, including advice on what forms you may need to use. However, Mead Library has available numerous tax guides for reference use or checkout.

Free tax help is available at Mead for the elderly, handicapped, or low-income people. The Wisconsin Department of Revenue representatives will offer personal tax assistance: basic questions about tax returns and help filling out Wisconsin Homestead tax forms. Register at the second floor Public Conference Room when you arrive.
Dates are: Tuesday, Jan. 19; Monday, Feb. 1; Tuesday, Feb. 16; Monday, March 1; Monday, March 15; Monday, March 29; Monday, April 5; and Thursday, April 15. Reps will be at Mead from 9 a.m. to 2:30 p.m.
For additional information or disability accommodations, call 459-3400, ext. 3422, at least a week in advance.
One is required to have an income under $49,000, required to have earned all 2009 income in Wisconsin, and required to be a legal U.S. resident.
Participants should bring a copy of 2009 Federal and State tax returns (if available), all W2s and 1099s, Social Security cards for you, your spouse and dependents, Wisconsin 2009 property tax bill OR rent certificate (signed by landlord), a voided check (for direct deposit.
Federal refunds take approximately 5-7 days if direct deposit is used, Wisconsin refunds take approximately 2 weeks if direct deposit is used, both take about 3-4 weeks without direct deposit.
There is absolutely no cost involved in this service. The returns are prepared at the Salvation Army, 710 Pennsylvania Avenue, Sheboygan, WI on Tuesdays/Thursdays 5:00 - 8:00 p.m. and Saturdays from 9:30 a.m. - 1:30 p.m. Additional dates are available by appointment only.
